cataract surgery management in eyes with extensive iridoschisis

چکیده
purpose: to demonstrate an approach to the problems of cataract surgery in extensive iridoschisis methods: four eyes of 2 patients with extensive iridoschisis had cataract surgery by the same surgeon using the techniques that resembled techniques for phacoemulsification in intraoperative floppy iris syndrome (ifis). we used low flow parameters on phacoemulsification, used super-cohesive ophthalmic viscosurgical devices (ovds), avoided iris stretching, inserted iris hooks, administrated topical atropine preoperatively, and injected intracameral epinephrine. results: iris fibrils remained immobile during the procedure, and despite the severity of iridoschisis, there was no iris attachment to phaco tip, iris billowing, or iris prolapse during phacoemulsification. none of the eyes developed posterior capsule rupture. conclusion: in severe iridoschisis patients, there are risks of aspiration of iris fibers and iris prolapse during cataract surgery. these might be prevented by combination of using super-cohesive ovds, avoidance of iris stretching, insertion of iris hooks, preoperative administration of topical atropine, and intracameral epinephrine. iranian journal of ophthalmology 201224(4):40-44 © 2012 by the iranian society of ophthalmology

متن کاملCataract Surgery in Eyes with Open Angles
Recent findings As a result of the high rate of cataract surgery, the impact on IOP continues to be the subject of multiple studies in different populations. Recent publications include those that distinguish patients with open angles from those with more narrow angles, as well as prospective analyses that address the impact of regression to the mean and other types of bias on the effect of pos...
متن کاملCataract Surgery in Compromised eyes
C ataract surgery in compromised eyes is one of the major challenges faced by ophthalmologists. Sometimes, despite perfect anatomic results, comorbidities within the eye can impair functional vision postoperatively. In order to achieve favorable results in these cases, a thorough intraoperative strategy is mandatory. Below I describe the preand postoperative regimen we use at the Academic Teach...
متن کاملCataract Surgery Eyes with Zonular Weakness
IOP PXF is the most common identifiable cause of openangle glaucoma (OAG).2 Roughly 20% of cases of this disease are associated with PXF, and approximately 30% to 50% of patients with PXF develop glaucoma. Surgeons have come to recognize that uncomplicated phacoemulsification has a beneficial effect on IOP in eyes with OAG, and this effect extends to eyes with PXF. In a series of more than 1,00...

D uring the last decade, the intraoperative use of various devices—including those for the shortand long-term support of the pupil, capsule, and zonules—has revolutionized the treatment of eyes with zonular laxity or focal zonular absence. Intraoperatively, surgeons use both temporary and permanent implantable devices as well as ophthalmic viscosurgical devices (OVDs). The combination of these ...

RATIONALE Iridoschisis is a rare eye disease. In this case report, we described the examination and diagnosis of a case of iridoschisis accompanied by secondary glaucoma. We also observed the effects of treating the patient with a combination of goniosynechialysis and cataract removal. PATIENT CONCERN A 67-year-old female patient presented with decreased vision in both eyes. An eye examinatio...
